Printed Fingerprint Card

FD-258 Fingerprint Card

Some agencies require a printed fingerprint card (FD-258) instead of the electronic Livescan submission.
Your agency may also have you choose your preferred method (fingerprint card or electronic Livescan submission).
First printed card is $25.00. Each additional card at the time of service is $5.00.

What to bring to your appointment:

Present a valid photo ID at the time of fingerprinting. Revoked, suspended or expired ID will not be accepted.

Examples of acceptable IDs include:

If you are obtaining fingerprints for the Department of Health and Human Services (DHHS), you will need to provide a fingerprint authorization form from your employer/company. This form will includes a 6-digit number specifically assigned to you that is required for processing.
