Printed Fingerprint Card

  • Some agencies require a printed fingerprint card (FD-258) instead of the electronic Livescan submission.

  • Your agency may also have you choose your preferred method (fingerprint card or electronic Livescan submission).

  • First printed card is $25.00. Each additional card at the time of service is $5.00.

A 2x2 photo is available for $10.00.

What to bring to your appointment:

Present a valid photo ID at the time of fingerprinting. Revoked, suspended or expired ID will not be accepted.

Examples of acceptable IDs include:

  • (Any) State Driver’s License

  • (Any) State ID

  • DMV Driver’s Permit

  • US Passport

  • Permanent Resident Card

  • Military ID

If you are obtaining fingerprints for the Department of Health and Human Services (DHHS), you will need to provide a fingerprint authorization form from your employer/company.   This form will includes a 6-digit number specifically assigned to you that is required for processing.
